A lot of NHS organisations have their own books with these certificates in. However, a sample copy which could be used is attached as an appendix in the document which recommends their use - if you go to the MHRA site (www.mhra.gov.uk) and look for Device Bulletin DB2003(05) entitled Management of Medical Devices prior to Repair, Service or Investigation, this is what you need.
