MRSA Bacteraemia Root Cause Analysis

Does anyone out there have a root cause analysis tool that they are currently using for investigating MRSA bacteraemia's? Also do most teams perform the root cause analysis themselves and then feedback to clinical teams or do you ask the clinical team to investigate the incident and then feedback the results to the Infection Control Team and what success's have you had with either option?

In the new MRSA guidelines (in the Journal of Hospital Infection, Vol 63, Supplement 1, May 2006) there is a "Fishbone cause and effect diagram" for MRSA transmission(not specifically bacteraemias).
